Care Connection Program

The Santa Fe County Board of County Commissioners and the CARE Connection partners have prioritized the need for increased availability of and access to behavioral health services (mental health and substance abuse). They have also identified the need for enhanced collaboration and linkage among providers to assure an improved continuum of care for substance abuse and other behavioral health services and diversion from inappropriate jail and hospital services.

The CARE Connection is a network of community providers, consumers, advocates, local government staff, and others. It was established to represent the interests of citizens with behavioral health needs and address the coordination and collaboration of services. Goals include increased access to behavioral health services and positive outcomes for persons in need of substance abuse and/or mental health services in Santa Fe County. These outcomes include increasing the capacity of treatment providers, stronger collaborative efforts between service providers, diversion from the emergency room, decreased detention in jails of persons with behavioral health issues and, most significantly, positive treatment outcomes for individuals screened, assessed, and referred by the CARE Connection.

The CARE Connection Advisory group meets every other month to plan, develop and implement assessment, date collection, case management, referrals and sobering services that are located at the County’s Health and Human Services Division Facility at 2052 South Galisteo Street in Santa Fe. Members of the CARE Connection make recommendations to Santa Fe County program staff and the Health Planning and Policy Commission (HPPC).

CARE Connection Assessment Center

Clinical assessments are conducted for people with mental health and/or substance abuse problems.  Trained therapists and case managers assist clients in accessing treatment and recovery support services.  The Assessment Center also administers a federal “Access to Recovery” (ATR) grant that can pay for services including:  substance abuse treatment, group/family/peer support, spiritual/pastoral guidance, transportation, acupuncture, massage, child care, and other services that help support treatment and recovery. Services are provided for residents of Santa Fe County or those who live within a 20 mile radius of the county line, who are income eligible and who have a substance abuse problem. 

There are some 35 community providers in the ATR network that clients can choose from for their treatment and recovery services. The Assessment Center is located at 2052 South Galisteo Street, next door to the Magistrate Court building.

Santa Fe County Sobering Center

The second program within the CARE Connection is the Santa Fe County Sobering Center.  The  Sobering Center is intended to function as a diversion from and an alternative to unnecessary interventions and detention by local law enforcement authorities and over utilization of emergency medical services.

The Santa County Sobering Center is designed as a harm reduction sobering program that provides a supported and controlled sobering experience for individuals volunteering to undergo an acute abstinence episode from alcohol and/or drugs for three to five days in a residential program setting.

The program is open to individuals 18 yrs. of age and older whose alcohol and/or drug use has resulted in law enforcement interventions and/or emergency medical services not requiring subsequent criminal charges or hospitalization, respectively.

The Sobering Center is a 24/7 sobering program that operates 365 days a year with a bed capacity of 15: five (5) beds are designated for the women’s dormitory and ten (10) are designated for the men’s dormitory.
