The County of Santa Fe currently pays 63% of the insurance health premiums and employees pay 37% of the insurance health premiums; for medical, dental, vision, and basic life and disability insurance. Presbyterian:
Under this plan, the employee is required to select and access care through a Primary Care Physician (PCP) who will assist you in coordination your health care through the Presbyterian net work. Emergency and Urgent Care Services are the only out of state services that are paid through this plan. You also have access to the Multiplan national network of providers through the PHS plan. This plan is a dual option PPO plan which provides choice and flexibility in benefits on a nation-wide basis. The employee is not required to select a Primary Care Physician, but out of pocket cost is higher. This is an open access plus plan PPO that provides choice and flexibility in benefits on a nation- wide basis. The employee is not required to select a Primary Care Physician, but out of pocket cost is higher. Medco Health Solutions, Inc.By enrolling in one of the medical plans, an employee is automatically covered under this prescription drug program. When selecting a Dentist, employees are guaranteed the lowest cost when selecting a PPO of New Mexico who accepts Delta Dental’s discounted fees and charges. Employees can also elect a dentist who does not participate in the PPO of New Mexico plan; however the cost may be higher when visiting a non-Delta dentist. When selecting a network Vision Service Plan, the employee will receive discounted fee and the lowest cost. This plan allows employees to receive $110.00 towards contacts, glasses and applies to the cost of lenses and fitting. Please refer to the summary plan description/benefits for full details. www.vsp.com Basic life and AD&D, Optional Term life and Family Dependent Term Life Insurance:This is a basic term life and accidental death and dismemberment insurance, and is offered to employees, spouses and dependents.
In addition, this plan also covers children, under the age of 25, Domestic Partners, and Domestic Partners children.
Supplemental Life:This is a basic term life and accidental death and dismemberment insurance, and is offered to employees who wish to elect an amount in excess of the basic life package. Newly hired employees may select an amount up to three times your annual salary (not to exceed 150,000). The maximum amount of life coverage offered is $400,000 with an underwriting approval. Short Term Disability:Employees enrolled in the basic tern life package will automatically be enrolled in the STD program. The maximum period under this plan is 150 days. Long Term Disability:After an employee exhausts Short Term Disability benefits, they will automatically be eligible for longer term disability coverage if they are still disabled. Cobra:This is a plan that allows eligible employees who separate from the County the opportunity to select continued health coverage for themselves and any other dependents under the plan. Flexible Spending:This program is a convenient way to pay for medical and dependent care expenses. When an employee enrolls, they decide how much to contribute to each account for the entire Plan Year. This money is deducted from the employees’ paycheck, pre-tax, in equal amounts, over the course of the plan year.
